Subsequent Repeat Petition

Before attempting a course a third time, this form must be approved by your program.

Complete this form only for courses offered in the School of Engineering, i.e. ENGR, BIOE, CSE, ENVE, ME or MSE. If you are petitioning for multiple courses, you will need to submit a separate form for each course.

Approval is not guaranteed. Subsequent course repeats are only approved in exceptional cases, and you should provide supporting documentation with your request.

You will be notified via email when a decision has been made. Please note that you might not receive a decision until after registration for the next term has begun. Get more information about School of Engineering repeat policies.

Please only submit one request per course. If you need to repeat two separate courses, please submit two subsequent repeat petitions.

 

Please be advised, that if you did not receive an email confirmation within 12 hours, then the appeal was not submitted, and you will need to file another appeal for consideration.

 

**Note: Petitions will be reviewed three times per semester: (1) Prior to the first day of courses (2) Before next term (Fall/Spring) Registration Window opens and (3) After the term has ended**

You must have earned "D+" or lower (No Pass) twice for this course to make this request. If you withdrew from the course, then that attempt does not count for the purpose of this form.
Courses hosted through the School of Natural Sciences (Math, Phys, etc,) or SSHA (Psy, Cogs, etc.) must be requested through that Schools webform.
When are you planning to repeat the course (example: Fall 2015, Spring, 2016, etc.)?

Third and/or subsequent attempts of a course are only approved under extenuating circumstances. In a separate, typed statement, please explain why you are requesting to repeat the course (besides the course being required for your intended major), including details as to why you have been unsusccessful in the previous attempts of the course, evidence of changes in your academic behavior and the goals that will lead to academic success. 

Please upload your statement and supporting documentation below. Incomplete submissions will not be considered.

Your statement should outline the factors that impeded your successful completion of the course in previous semesters and why an exception should be made. Having the course as a requirement for your intended major is not an extenuating circumstance that would warrant an exception.
